What is the etiquette to knocking on the door?

My boss schedule a meeting in his office with several people. I arrived first and walked right into his office at sat down. He said that I should knock before entering. I was a bit surprise being that it was a schedule meeting. What is the proper etiquette? I usually knock when I interrupt someone or its an emergency.

Whether it is a scheduled meeting or not, you always knock or say “May I come in?”. You arrived early for the meeting and he might not have been ready to have people in his office.
